Yes there are formats and guidelines set out by the School of Graduate Studies (SGS) as well as those customized to individual school. However, the general sections that must be included in the draft concept or proposal will cut across Schools. I have attached a fair summary of sections you need to have in the draft. I hope that help you develop a concept.
